Returning Athletes Only

Here are each of the pages you need to complete in order to get cleared for participation during the 2015-16 season:

 

**Team Participation Agreement:  Must be completed for every sport every season

 

**Student Alternate Transportation Form:  For students that will either be transporting themselves and/or will be transported by an adult other than a parent/guardian including Coach.

 

Student Personal Auto Usage Form:  For students that will be transporting themselves, requires a copy of current CDL and insurance declaration page in order to be complete.  

Reminder:  if you are filling this form out, you will also need the Student Alternate Transportation Form.

 

**Concussion and Head Injury Information Sheet:  Required for all athletes prior to participation, response to California law that went into effect on Jan 1, 2012.

 

**Sudden Cardiac Arrest Information Sheet:  Required for all athletes prior to participation, California law 2015.

 

**Physical Form:  required every year, must be dated after May 1, 2015

 

**Emergency Form:  Maybe picked up from Coach or in the Athletic Office

 

** = REQUIRED TO BE CLEARED, NO EXCEPTIONS!!

Spring 2016

 

All Athletes must be totally cleared before starting spring practices/tryouts on February 8th.  The dead period is January 18th – February 7th.

 

Before practicing, each student-athlete must have:

 

1.  All Sheldon High School books, uniforms and equipment returned and no outstanding bills or fines at Sheldon High School.

2.  A minimum GPA of 2.0 on the Term 2 P4 report card.

3.  Physical (after May 1, 2015)—completed and signed by parent and doctor.

4.  EGUSD Athletic Handbook— all completed and signed by parent and athlete.

      a) Team Participation Form **

             - Must fill one out every season for every sport

        b) Student Alternate Transportation Form **

           - For students who will get transportation from adult other than parent

            - required for all athletes, no exceptions

         c) Student Personal Auto Usage Form

             - For students to drive self to event

              - NEEDS copy of CDL and declaration page of insurance to be complete

              - requires Student Alternate Transportation Form also

          d)  Concussion and Head Injury Info Sheet **

                - California law

          e)  Sudden Cardiac Arrest Info Sheet**

              - New California law

5.  Emergency Information Form—completed and signed by parent.

6.  Mandatory New Athlete and Parent Meeting—attended by athlete and parent when first starting athletics at SHS—the month and date that you previously attended this meeting is asked for on page 15 form in the Athletic Handbook.

 

*New athletes must attend the Mandatory New Athlete and Parent Meeting

They must have completed the required requirements #1-3 above and must attend the Mandatory New Athlete and Parent Meeting on February 2, 2016 at 7:00pm in the SHS Cafeteria, both the athlete and a parent must be present at the meeting.

Wrestling lost to Davis 39-36

 

Upcoming Events:

Wednesday:

Men’s Basketball will host Davis. Frosh will start at 4:00, JV will start at 5:30, and Varsity will start at 7:00.

Women’s Basketball will travel Davis. JV will start at 5:30 and Varsity will start at 7:00.

Friday:

Men’s Basketball will travel to Jesuit. Frosh will start at 4:00, JV will start at 5:30, and Varsity will start at 7:00.

Women’s Basketball will host Saint Francis. JV will start at 5:30 and Varsity will start at 7:00.

Saturday:

Wrestling will travel to Lincoln HS for the Section Championships.

 

Announcement:

Baseball tryouts will be on Monday at 8:30 AM.

Softball tryouts will be on Monday at 9:30 AM.

Soccer tryouts will be on Monday. Varsity will be from 9:30-11:00 and JV will be from 11:00 to 1:00.

Track tryouts will be on Monday at 3:30 on the track.

Football will have a parent meeting on Thursday the 11th at 7:00 PM in the cafeteria.

 

 

All Spring athletes, it’s time to get cleared to participate.  Spring practices begin Feb. 8th.

 

All new athletes must attend a New Parent and Athlete Meeting.  This is a required meeting, both athlete and parent must attend in order to receive the required paperwork.

 

All returning athlete please visit the Athletics page on the SheldonHuskies.com website to get required paperwork.  See Ms. Aguilar or Coach for an emergency form.

 

Remember you need a current physical to participate!

 

The Athletic Office will be open during both lunches.
