Returning Athletes Only

Here are each of the pages you need to complete in order to get cleared for participation during the 2016-17 season:

 

**Team Participation Agreement:  Must be completed for every sport every season

 

**Student Alternate Transportation Form:  For students that will either be transporting themselves and/or will be transported by an adult other than a parent/guardian including Coach.

 

Student Personal Auto Usage Form:  For students that will be transporting themselves, requires a copy of current CDL and insurance declaration page in order to be complete.  

Reminder:  if you are filling this form out, you will also need the Student Alternate Transportation Form.

 

**Concussion and Head Injury Information Sheet:  Required for all athletes prior to participation, response to California law that went into effect on Jan 1, 2012.

 

**Sudden Cardiac Arrest Information Sheet:  Required for all athletes prior to participation, California law 2015.

 

**AT Consent To Treat:  required for athletic trainer to treat athletes

 

**Physical Form:  required every year, must be dated after May 1, 2016

 

**Emergency Form:  Maybe picked up from Coach or in the Athletic Office

 

** = REQUIRED TO BE CLEARED, NO EXCEPTIONS!!

Results:

 

Last Friday our cross country team competed in the Lodi Flame Invitational:

Our Varsity Boys team took 4th place, and was led by Matt Simons, who finished in 7th place overall on the 3 mile course.

Our Varsity Girls team took 7th place.  Lauren Windmiller & Perla Gonzalez finished in 7th & 8th place overall in the race. 

Our JV Boys team took 2nd place, and was led by Ben Lucca who took 1st place in the 2 mile race

Our JV Girls team took 2nd place, and was led by Amber Hughes, who finished in 3rd place overall.

Our Freshmen Girls team also took 2nd place, and was led by Madi Noll & Carmen Iturriaga who finished in 1st & 2nd place respectively.

Our Freshman Boys team took 4th place, and was led by Miguel Ayala & Patrick McGonigle,

Our Sophomore Boys team took 8th place, and was led by Isaac McInnis

 

Great job Cross Country!!

Women’s tennis will travel to Valley High School for a pre-season match. Start time is 3:30 p.m.

Friday

Football will host Casa Roble for their first pre-season home game. JV Start time is 5:00 p.m. and Varsity start time is 7:15 p.m.

Saturday

Cross Country will travel to Vacaville to compete in the Lagoon Valley Classic.

Varsity football defeated Woodcreek High School 46-21 on Friday. Great job football! Way to kick off the year with a win!!

WHAT IS AN ALL SPORT PASS?

All - sport passes are for parents and adult family members of our students who wish to attend Sheldon athletic events.

An All - sport pass will give the holder free admission to all home athletic events with the exception of play-off games.
