FREE Athletic Physicals

The Sheldon Football Boosters and the SHS Athletic Department are teaming up with Methodist Hospital to once again host free athletic physicals on Saturday May 12th in the Aux Gym.  Doors will be open from 9am - 1pm.  Please bring the physical form below signed by a parent/guardian or bring your parent.  YOU MUST HAVE PARENT PERMISSION TO GET A PHYSICAL!

Returning Athletes Only

Here are each of the pages you need to complete in order to get cleared for participation during the 2012-13 season:

**Team Participation Agreement:  Must be completed for every sport every season

**Student Alternate Transportation Form:  For students that will either be transporting themselves and/or will be transported by an adult other than a parent/guardian including Coach.

Student Personal Auto Usage Form:  For students that will be transporting themselves, requires a copy of current CDL and insurance declaration page in order to be complete.  

Reminder:  if you are filling this form out, you will also need the Student Alternate Transportation Form.

**Concussion and Head Injury Information Sheet:  New this year, required for all athletes prior to participation, response to California law that went into effect on Jan 1, 2012.

**Emergency Form:  Maybe picked up from Coach or in the Athletic Office
