Returning Athletes Only

Here are each of the pages you need to complete in order to get cleared for participation during the 2014-15 season:


**Team Participation Agreement:  Must be completed for every sport every season


**Student Alternate Transportation Form:  For students that will either be transporting themselves and/or will be transported by an adult other than a parent/guardian including Coach.


Student Personal Auto Usage Form:  For students that will be transporting themselves, requires a copy of current CDL and insurance declaration page in order to be complete.  

Reminder:  if you are filling this form out, you will also need the Student Alternate Transportation Form.


**Concussion and Head Injury Information Sheet:  Required for all athletes prior to participation, response to California law that went into effect on Jan 1, 2012.


**Physical Form:  required every year, must be dated after May 1, 2014


**Emergency Form:  Maybe picked up from Coach or in the Athletic Office

Why ASB for $30? ($13 goes to athletic fund).  ASB cardholders get into all home athletic events (excluding playoffs or foundation games) for free; get a $1-2 discount at away Delta League events and discounts at dances, performances, etc.  This could add up to over $200 in savings.

NCAA Clearinghouse

All seniors who plan to compete in athletics at a Division I or II NCAA university or college next year must register with, and be cleared by, the NCAA Clearinghouse.  You now register online.  If you have not done so as of yet, do it today at or link from NCAA’s website:

Sport Shorts:


JV Softball beat Franklin 12-3 on Friday. Vanessa Porto, Taliyah Miles and Jewel Unga each had 2 hits.

Varsity Softball also beat Franklin 16-4. Sarah Brown had 3 hits with 2 triples and 4 RBI's. Jessica Scott had 3 hits with 1 double and 3 RBI's. Juju Malana had 3 hits with a triple. Maddie Ayers went 2 for 3. Jordan Fines went 2 for 3 with a double.  Maci Fines and Sarah Fukushima both hit home runs.






Girls Soccer will host Grant

Husky Club Wrestling Tuesdays and Thursdays in the Auxiliary Gym from 6-8.  Anyone interested in Wrestling come out and participate.

At, Sheldon High School, we are proud of the tradition of excellence that has become the standard for our teams and the athletic program, and we are dedicated to maintaining these standards of excellence in the classroom and the competitive arena.  In particular, we are especially committed to promoting excellence through good sportsmanship and positive support for all of our student-athletes, coaches and staff as well as for our opponents and officials.
