Returning Athletes Only

Here are each of the pages you need to complete in order to get cleared for participation during the 2016-17 season:

 

**Team Participation Agreement:  Must be completed for every sport every season

 

**Student Alternate Transportation Form:  For students that will either be transporting themselves and/or will be transported by an adult other than a parent/guardian including Coach.

 

Student Personal Auto Usage Form:  For students that will be transporting themselves, requires a copy of current CDL and insurance declaration page in order to be complete.  

Reminder:  if you are filling this form out, you will also need the Student Alternate Transportation Form.

 

**Concussion and Head Injury Information Sheet:  Required for all athletes prior to participation, response to California law that went into effect on Jan 1, 2012.

 

**Sudden Cardiac Arrest Information Sheet:  Required for all athletes prior to participation, California law 2015.

 

**AT Consent To Treat:  required for athletic trainer to treat athletes

 

**Physical Form:  required every year, must be dated after May 1, 2016

 

**Emergency Form:  Maybe picked up from Coach or in the Athletic Office

Spring Sports are coming:

 

Spring practices begin on or after Monday February 8, 2016.  All athletes must be cleared before practicing. 

 

  1. Student-athletes need 2.0 minimum GPA on their Term 2 report card and no F’s on first progress report of Term 3.

 

  1. A physical exam must have been taken after May, 2016 and have a form on file in the athletic office.

 

  1. RETURNING ATHLETES that played this fall or winter come by athletic office and pick up a second or third sport form.  Complete and return.

 

  1. RETURNING ATHLETES that played last year but not this fall or winter, you must complete and return all required paperwork:  EGUSD Athletic Handbook pages, Risk Acknowledgement and Liability Waiver, and the Emergency Information Form. All Forms are available on line on the athletics page of www.sheldonhuskies.com except the Emergency form which can be picked up at the athletic office or from your coach.

 

  1. ALL NEW ATHLETES must attend the New Athlete and Parent Mandatory Meeting with a parent or guardian which will be held on Monday , January 30, 2017 from 7:00 p.m. – 9:00 p.m.  The required paperwork, except the physical form, will be handed out and completed at this meeting.  If you do not attend this meeting, please see Denise Aguilar, Athletic Director, to make an appointment.

Friday

Varsity Wrestling will travel to Memorial Auditorium to compete against Tim Brown through Saturday.

Women’s basketball will host St. Francis. JV start time is 5:30 p.m. and Varsity start time is 7:00 p.m.

Men’s basketball will travel to Jesuit. Freshmen start time is 4:00 p.m. JV start time is 5:30 p.m. Varsity start time is 7:00 p.m.

Saturday

Varsity Wrestling will travel to Memorial Auditorium to compete against Tim Brown.

JV Wrestling will travel to Albany High School for the Albany Invite.

Men’s Varsity basketball will travel to Edison High School in Fresno to compete in the Fresno Challenge. Game times are 5:30 p.m. and 7:00 p.m.
