Returning Athletes Only

Here are each of the pages you need to complete in order to get cleared for participation during the 2014-15 season:

 

**Team Participation Agreement:  Must be completed for every sport every season

 

**Student Alternate Transportation Form:  For students that will either be transporting themselves and/or will be transported by an adult other than a parent/guardian including Coach.

 

Student Personal Auto Usage Form:  For students that will be transporting themselves, requires a copy of current CDL and insurance declaration page in order to be complete.  

Reminder:  if you are filling this form out, you will also need the Student Alternate Transportation Form.

 

**Concussion and Head Injury Information Sheet:  Required for all athletes prior to participation, response to California law that went into effect on Jan 1, 2012.

 

**Physical Form:  required every year, must be dated after May 1, 2014

 

**Emergency Form:  Maybe picked up from Coach or in the Athletic Office

 

** = REQUIRED TO BE CLEARED, NO EXCEPTIONS!!

UPCOMING EVENTS:

Huskies Ready for D1 Section Track Championships

 

The Sheldon boys took a major hit during Wednesday’s Division I Section track and field prelims when defending 400 meter champion and section meet record holder (46.99) Cameron Stone reinjured his hamstring.  The Oregon bound senior will not compete in tonight’s 400 meter dash and 1600 meter relay championship finals, ending a stellar high school career which saw him qualify for two consecutive state championships including last season when he led the boy’s team to a third place finish in the 1600 meter relay.

On a more positive note the Husky boys sped to the day’s fastest qualifying time in the 400 meter relay when the team of Stephon Rhodes, Marcus Beloney, Elias Brown and Erick Spivey clocked 42.07. Beloney, who owns Nor Cal’s second best 400 meter mark, behind Stone at 48.12, advanced to tonight’s championship final in that event. Sophomore, Derek Dokter also advanced to the consolation final in the 800 meters. Senior Jonathan Perry continues to impress winning his heat of the boys 110 High Hurdles in a school record 14.72 to qualify for tonight’s championship final.

On the girl’s side, senior Amari Jones and freshman Tai McDonald will both compete in the consolation final 100 meter dash, while McDonald is a finalist in the championship 200 meter dash event.  Jones will also compete in the consolation final 200 meter dash. Rounding out the girl’s competitors, junior thrower Janessa Moses goes in the discus throw.

The top eight finishers from tonight’s championship final automatically advance to next week’s Master’s Prelims which the top four qualifiers from the consolation final will also advance. Running events at Elk Grove-Sheldon Community stadium begin at 6:00 p.m. and field events kick off at 1:00 p.m. Go Huskies!
