Returning Athletes Only

Here are each of the pages you need to complete in order to get cleared for participation during the 2014-15 season:

 

**Team Participation Agreement:  Must be completed for every sport every season

 

**Student Alternate Transportation Form:  For students that will either be transporting themselves and/or will be transported by an adult other than a parent/guardian including Coach.

 

Student Personal Auto Usage Form:  For students that will be transporting themselves, requires a copy of current CDL and insurance declaration page in order to be complete.  

Reminder:  if you are filling this form out, you will also need the Student Alternate Transportation Form.

 

**Concussion and Head Injury Information Sheet:  Required for all athletes prior to participation, response to California law that went into effect on Jan 1, 2012.

 

**Physical Form:  required every year, must be dated after May 1, 2014

 

**Emergency Form:  Maybe picked up from Coach or in the Athletic Office

Spring practices begin Monday, February 9, 2015.  All athletes must be cleared before practicing.  Clearance requires the following:

  1. Student-athletes need 2.0 minimum GPA on their Term 2 report card and no F’s on first progress report of Term 3.
  2. A physical exam must have been taken after May, 2014 and have a form on file in the athletic office.
  3. RETURNING ATHLETES must complete and return all required paperwork:  EGUSD Athletic Handbook pages, Risk Acknowledgement and Liability Waiver, and the Emergency Information Form. Forms are available on line on the athletics page of www.sheldonhuskies.com.
  4. NEW ATHLETES must attend the New Athlete and Parent Mandatory Meeting with a parent or guardian which will be held on Tuesday , February 3, 2015 from 7:00 p.m. – 9:00 p.m.  The required paperwork, except the physical form, will be handed out and completed at this meeting.  If you do not attend this meeting, please see Denise Aguilar, Athletic Director, to make an appointment.

All-Sport Passes

These passes are on sale at the home basketball games and wrestling matches. The cost for one is $30 and a special family rate of $90 for an immediate family of 4. This pass gets you free admission to all home athletic events except for CIF playoffs and foundation games. You can save money and time as the general admission for basketball is $7and wrestling $6. Remember that you are not guaranteed a seat if it’s a sell-out.

Why ASB for $30? ($13 goes to athletic fund).  ASB cardholders get into all home athletic events (excluding playoffs or foundation games) for free; get a $1-2 discount at away Delta League events and discounts at dances, performances, etc.  This could add up to over $200 in savings.

NCAA Clearinghouse

All seniors who plan to compete in athletics at a Division I or II NCAA university or college next year must register with, and be cleared by, the NCAA Clearinghouse.  You now register online.  If you have not done so as of yet, do it today at www.ncaaclearinghouse.net or link from NCAA’s website:  www.ncaa.org.

HUSKY NATION HAPPENINGS

Sport Shorts:

 

RESULTS:

Freshman Boys Basketball defeated Pleasant Grove 56-44.  Jaquane Badger scored 22 and Marcus Robinson scored 18 points.

JV Boys Basketball also defeated Pleasant Grove 73-63.  Freshman Dale Curry scored 22 points and 5 different Huskies scored in double figures.

Varsity Boys Basketball lost to Pleasant Grove on Friday night 65-61, but defeated El Cerrito 62-61 on Saturday.

Wrestling  Ethan Brewer placed 4th at Tim Brown, one of the toughest tournaments in Northern California. Meng Xiong took 3rd and Madison Quint and Abren Lang took 7th at Albany.
